Tapentadol ER Dosage

The individualized dose of tapentadol ER you are prescribed will depend on factors such as how you respond to the medication, the severity of your pain, and other medical conditions you may have. Generally, the typical amounts range from 50 mg to 250 mg every 12 hours. Your dosage may be adjusted as necessary, up to a maximum amount of 500 mg total per day.

An Introduction to Your Dosage of Tapentadol ER

The dose of tapentadol ER (Nucynta® ER) your healthcare provider recommends will vary, depending on a number of factors, including:
 
  • The type and severity of your pain
  • How you respond to the medication
  • Other medications you are taking
  • Other pain medications you have tried
  • Other medical conditions you may have.
 
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.
 

Tapentadol ER Dosing Guidelines for Pain

As with many opioid pain relievers, there is variation in the dosage of tapentadol ER that can be tolerated and that will effectively relieve pain. This variation can depend on several factors, such as the severity of pain and the person's past history of opioid use (people become tolerant to the effects of opioids over time). In general, typical tapentadol ER dosages range from 50 mg to 250 mg every 12 hours.
 
In many cases, people need to take short-acting "rescue" painkillers in addition to tapentadol ER to help control "breakthrough" pain. Importantly, short-acting tapentadol or any medication that contains tramadol (such as Rybix™ ODT, Ryzolt®, Ultram®, Ultram® ER, or Ultracet®) should not be used in combination with tapentadol ER. A different type of medication that does not contain tapentadol or tramadol should be used for breakthrough pain.
 
If you have liver disease, your healthcare provider may recommend starting with a low dose, as you may not handle tapentadol ER as well as normal.
 
It is a common "off-label" practice for long-acting, twice-daily narcotics to be prescribed on a more frequent basis (typically every 8 hours, instead of every 12 hours) for people who find the medication does not last long enough. No matter how often this medication is taken, the maximum total daily dose of tapentadol ER is 500 mg.
