Fentanyl Dosage

Before prescribing a fentanyl product, your healthcare provider will consider the severity of your pain and other medications being taken, among other things. There is a significant variation in the dosage of fentanyl that can be tolerated and that is effective at relieving pain. In most cases, your healthcare provider will start you on a low dose and gradually increase the amount as necessary.

An Introduction to Your Dosage of Fentanyl

The dose of fentanyl (Abstral®, Actiq®, Duragesic®, Fentora®, Lazanda®, Onsolis®, Subsys®) your healthcare provider recommends will vary, depending on a number of factors, including:
  • The particular fentanyl product
  • The type and severity of your pain
  • How you respond to the drug
  • Other medications you are taking, including other pain medications
  • Other medical conditions you may have.
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.

Initial Fentanyl Dosing Guidelines

Fentanyl doses are highly individualized and will differ among the various fentanyl products. These products are not interchangeable and their doses should not be considered equal.
For some fentanyl medications, the initial dosage will be the same for most people. For other products, the initial dose will depend on other factors, such as other pain medications taken.
If your initial dose adequately relieves your pain, you will likely continue with this amount. If your pain persists, however, the dosage will be gradually increased to one that effectively relieves pain without causing significant side effects.
How quickly the dose is increased will also differ among fentanyl products. With long-acting fentanyl medications, it may take several days to feel maximum pain relief. Therefore, increases are normally made after three to six days (or longer). In contrast, doses of short-acting fentanyl medications may be increased with each breakthrough pain episode, until an effective dose is found.
Because fentanyl dosages are highly individualized and there is a significant difference between the various products, it is important to refer to the specific dosing instructions for your particular medication.
